Newcomer Information Guide (English)

Recently many new immigrants are applying for the Permanent Resident (PR) card. Due to the complexity of application form content and the application procedures, we have compiled the following reminders as a quick reference guide:

1.  You must complete the application form with a black pen.

2. Item 2: If your present name is different from the name that appears on your permanent residency document, you will have to provide supporting legal documentation that accounts for a legal change of name.

3. Item 8: Chinese eye color is brown in most cases.

4. Item 13 & 14: You should list all home, work and school addresses from the last 5 years since you became a landed immigrant. Any information prior to those 5 years should not be included.

5. Item 15: List all your absences from Canada during the past 5 years, including travel absences.

    You must have lived in Canada for a total of 730 days during the past 5 years, to maintain your permanent resident status. If you were absent from Canada with good reasons accepted by Citizenship and Immigration Canada (i.e. accompanying a family member who is a Canadian citizen; employed by a Canadian business or in the public service of Canada or of a province; accompanying a family member who is a permanent resident of Canada and who is employed by a Canadian business or in the public service of Canada or of a province; or with valid Returning Resident Permit), you must provide the reasons and supporting documentation. (i.e. proof of family relationship, copy of employment letter, Returning Resident Permit, etc.)

    6. Item 16: You should fill out the place where you first entered Canada with a permanent resident status (found on the entry stamp on your passport).

    7. Section D: Signature of applicant

- An application made by an individual who is less than 14 years of age must be signed by one of the individual’s parents or the individual’s legal guardian.

- An application made by an individual between 14 and 18 years of age must be signed by the applicant and one of his/her parents or the individual’s legal guardian.

    8. Section E: Complete this section only if you are applying to replace a lost, stolen or destroyed PR card. Do not fill out this section if this is your first time applying.

    9. Section F & G: Guarantor must complete section F. If you are unable to find a guarantor, you must find a qualified official who has the authority to administer an oath to complete Section G.

    10. Applicant must bring passport, landing papers (or other IDs) and photographs to the office of guarantor or qualified official for declaration procedures.

    11. Applicant must provide 2 photos. The photos must have a finished size of 35mm X 45mm; be either black and white or color; bear the name and address of the photo shop, and the date the photo was taken directly on the back of the photo; and must allow sufficient space on the back for the signature of a guarantor (or qualified official).

    12. Applicant must provide a photocopy of passport and landing papers (or other IDs). Parents should also provide a photocopy of your child(ren)’s birth certificate. All photocopies must be certified by a guarantor (or qualified official) that they are photocopies of the original documents.

    13. The fee for processing your application is $50. Payments can be made at most financial institutions, i.e. banks. If you are submitting more than one application(i.e. applications for whole family), you must send one receipt to cover all applications. You must complete all the "Payer Information" sections on the reverse of the receipt.

    14. You must complete the Supplementary Identification Form (IMM5455) and sign within the Client Signature box. Be careful to sign only inside the white box in black ink. Your client ID is an 8-digit number on your landing paper; please note that not everyone has this number.

最近許多新移民都紛紛申請楓葉咭,但申請表格內容及手續甚為繁複,下列是
填寫楓葉咭申請表格時需注意事項,提供大家作為參考﹕

一。    填寫時必須用黑筆。

二。    2 欄 - 如來加後曾改名者,需要提供換名之證明文件。

三。    8 欄 - 中國人眼晴顏色大部分是褐色(brown)。

四。    13 及 14 欄填寫你過去五年住址及工作或學校名稱地址,是移民以
        後計算,移民之前的資料不需要填寫。

五。    15 欄填寫你過去五年曾離開加拿大之所有日期及資料,旅行出境資
        料也應填寫。

        在過去五年內,你需在加住滿 730 天,方能繼續保持加國居民身份。
        假如你曾離開加拿大,但有移民部可接受的合理原因(例如﹕陪同已
        成為公民的家庭成員,受雇於加國公司,陪同受雇於加國公司之已成
        加國永久居民家庭成員,或持有效回加紙)請在適當的空格上說明原
        因及提供證據(如親屬關系,雇用書,回加紙等之公證副本)。

六。    16 欄填寫你以永久居民身份進入加國時的入境地點(在護照上面的
        入境蓋印應有說明)。

七。    D 欄申請人簽名 - 十四歲以下小童由家長或監護人代簽
	               - 十四到十八歲需要申請人及家長或監護人簽名 

八。    E 欄是申請補領遺失楓葉咭之用,初申請者不用填寫。

九。    擔保人需在 F 欄填上所需資料。如新移民找不到擔保人則要找監誓
        官幫助宣誓,申請人亦需要填上 G 欄所需資料。

十。    申請人需帶同護照及移民紙(或其他有效身份證明文件)及照片到擔
        保人或監誓官處辦手續。

十一。  照片需要兩張 35 mm X 45 mm 大小,黑白或彩色,但必須印上照相
        館名字,地址和拍照日期,並需預留空間寫上申請人名字及給擔保人
        或監誓官簽名。

十二。  申請人需複印護照及移民紙(或其他有效身份證明文件),如有小童
        亦需複印小童之出生證明,再找擔保人或監誓官證明與正本相同 。  

十三。  費用每人 50 元,到銀行付錢,一家人只需一張收據。收據背面各欄
        也都需要填寫。

十四。  最後填好輔助證明表格(Supplementary Identification Form)。 
        你的編號 Client ID 是移民紙上的一組 8 位數字。不是每個人都有
        此編號。記緊要簽名,用黑筆,不可超越白色部份 。

注意﹕ 已經入籍成為加國公民者不需要申請楓葉咭。在不久將成為公民者,
        可以不申請。一切資料以加拿大移民及公民部所發報資料為準,如有
