When applying for a Canadian masters record, click on the field event form or on the track event form ,print it, fill in the required information, and mail it. Canadian masters record application forms should be sent by registered mail, and you should make copies before mailing the originals. Do NOT send the originals to the CMAA office address but to the CMAA Records Director who manages the records for your event. Their names and e-mail addresses are listed under 'CONTACTS' on the C.M.A.A. web page.
